The Air Force has released the police report associated with a fatal crash that occurred at Offutt Air Force Base.
The incident occurred on November 13th of last year, but The Salty Soldier was not able to find any public information or news related to the incident at the time.
According to the Air Force Security Forces report, Retired Master Sergeant Ronald Farnsley was driving his new 2023 Toyota Tundra Platinum edition when he crashed into a Valero gas pump at an AAFES Shoppette.
They did not speculate the speed he was driving when the crash occurred but images of the scene show he completely removed the pump from its foundation and caused severe damage to his vehicle.
The crash was so severe the truck’s front wheel and strut were completely broken from the truck and cracked.
The front of the vehicle also suffered damage and caused the truck’s air bags to deploy.
A witness who heard the accident said they were the first to arrive on the scene and found Farnsely unresponsive with his eyes open.
“I drove up to the wreck and called 911, then approached the driver side window and asked if the occupant was okay, with no response given,” they said.
They claim first responders arrived shortly after, at around 12:05 AM.
According to the 55th Security Forces Squadron, Offutt Air Force Fire Rescue removed Farnsley from the driver seat and immediately began CPR.
After being transported to the University of Nebraska Medical Center, he was pronounced dead at 1:16 AM.
Security Forces Airmen detailed where and how Farnsely’s truck impacted the gas pump but did not list an inventory of items found or any suspected cause of the crash.
Photos from the scene show the vehicle contained a cell phone and two soft coolers in the back seats.
